It's important. It helps cover liability while your child participates in 4-H.
A youth becomes a "4-H member" upon completion of an individual enrollment form and a 4-H medical release form (requires notarization) which must be on file in the county Extension Office. A youth participates in 4-H by belonging to a 4-H club or any recognized unit of membership in Jackson County.
